Hi there, this Q has cropped up a few times. These last answers are a few months old, but maybe some info/links still useful?

This would suggest that if you are from one of 15 countries they have started it again http://www.mip.gov.mm/visaonarrival/
However, I'd check with your local consulate as I wouldn't particularly trust the reliability of the above link, even if it is the official one. almost 4 years ago
Update 5 May 2013: You can get visa on arrival with travel agent. I did last March (2013). We only need to pay service fee 30 usd/ 1 pax and pay stamp fee at Yangon airport (30 usd). Total 60 usd and wait 15 days for approval letter. Here is more detail about visa on arrival in Myanmar http://visa2myanmar.com/faq/
