How to tell if something has been sitting in the vendors cart for a few more hours that it should have?

Just don't buy anything pre-prepared, ask them to cook it fresh for you... if they won't, go to another vendor, or offer to pay a little bit extra for freshly cooked.
Also, pick the busy street vendors - locals will know which are the good ones and they stay away from the bad ones. High turnover means there's less chance the food has been sitting around.
Use anti-bacterial hand wipes on the cutlery they give you (because it's usually washed in tap water - no thanks). The same goes for the cups/drinking glasses, so stick to bottled drinks. about 7 years ago
Agree with Phil. You will recognise the popular ones by the queue of locals. Make sure its cooked infront of you. Be adventurous and dont worry about what it is - rat, cat or bat - but enjoy the whole adventure.
If I am in an area for a couple of days then you get to see if the same vendor comes back to their spot each day and if they are busy. If yes and yes, then go to that one. My choice is usually for the one that has a stool that you can plonk down on and eat whilst you crowd watch. about 7 years ago
